So how do I go about getting rid of my last name everywhere?
Email addresses, insurance forms, work stuff, Drivers liscence....

I went in this order:
First the notarized copy of the signed marriage license.
Then social security and drivers license.
Work and health insurance was a 1 stop shop for me. The new email address at work was a bit painful for authorities, but it was short-lived.
Here's the fun part: Bank accounts, credit cards, bills...oh boy.
I still haven't changed everything. Checks still come to my maiden-name and the bank said they are ok since it is a legal alias.
I didn't get a passport until after I got married.
I think I got a guide sheet with the name change form as well.
